Humeral Nail System Surgical Technique

The Arthrex short humeral nail is intended to treat simple 2 to 3 part fractures of the proximal humerus. The short humeral nail features threaded holes proximally that accept five millimeter cancellous screws, a six degree lateral bend, 3.5mm screws distally, and a self reaming trocar tip. Simplified color coded instrumentation allows for an efficient, repeatable series of steps to ensure proper nail implantation. There are two acceptable entry points for the nail. One is just medial to the greater tuberosity, or in the center of the humeral head, straight down the humeral shaft. Begin by advancing the 3.2mm guide pin into the desired entry point, and utilize the 10.5mm entry reamer over the entry wire. The cannulated awl may also be used with gentle rotations to gain entry to the humeral canal. If reaming is necessary, utilize a ball tip, guidewire and flexible reamers. The ball tip guidewire must be removed prior to nail insertion, as it will not pass through the short humeral nail. Attach the nail to the targeting arm by inserting the holding bolt and aligning notches at the top of the nail and the connection point of the targeting arm. Tighten the holding bolt using the 5.0mm hex driver until the nail is securely fastened. The curvature of the nail should go towards the targeting arm once attached. Tighten the targeting jig onto the targeting arm by spinning the thumb wheel until tight. Advance the nail into the proximal body of the humerus through the entry portal. Gentle rotations may be needed to aid entry into the canal. Depth markings at the end of the targeting arm will help show the depth of the nail on a radiograph. The first line is two millimeters countersunk and the second line is five millimeters. The K-wires may also be used through the small hole in the in the targeting jig to indicate the top of the nail accurately. Utilizing the screw sheath and trocar. Enter through the targeting holes marked with an S and advance the sheath to the bone. Remove the trocar and thread in the yellow 3.5mm drill sleeve. Advance the 3.5mm. Drill bit through the drill sleeve until just inside the subchondral bone. Making sure not to drill into the joint space. Calibrations on the drill bit will indicate the depth of the screw needed. Measurement only corresponds to the threads of the screw and does not include the head of the screw. A hook tip depth gauge can also be used for measurement. Once the 3.5mm drill guide is removed. Insert the 5.0mm cancellous interlocking screw through the screw sheath using the 3.5mm hex driver and t-handle until the head of the screw is down to the bone. Repeat drilling steps for the remaining proximal screws where needed for distal 3.5mm screws. Insert the screw sheath and trocar into the distal targeting holes and advance the cannula to the bone while taking care to protect neurovascular structures. Remove the trocar and thread in the green 2.5mm drill sleeve. Advance the 2.5mm. Drill bicortical not advancing too far through the medial cortex and injuring soft tissue. The 2.5mm drill is calibrated to allow for quick measurement of screw size. The hook tip depth gauge may also be used. Once the 2.5mm drill guide is removed. Insert the desired length of the 3.5mm screw using the 2.5mm hex and t-handle until the head of the screw is flush to the bone. Repeat the process for the remaining distal screw. Verify all screw lengths on the radiograph and remove the targeting arm and jig.
